Support COORDINATION

Our staff have extensive experience and competent knowledge to develop and monitor supports and programs according to the needs of people with disabilities and within a legislative and ethical framework to ensure the provision of high quality service delivery which supports the rights and interests of people with disabilities.

Our support coordination team:

  • Collaborative Approach– our team works with all parties concerned in order for you to achieve yourpersonal goals.
  • Your choice– our team will support you the best way they could to give you a wide choice of service providers, wherever you are and whatever you need
  • Community Inclusion– to help you make the right choices to access and be part of your local community.
  • Good listener– look for someone who is committed to working closely with you, understanding your needs, and responding to you quickly and professionally.

supported ACCOMMODATION

All requests for supported accommodation are made by contacting our Referral, Intake, Information & Assessment team (RIIA) via email: hcsintake@halesupport.org.au or call them on 02 9625 5076.

The RIIA team maintains one Register of Requests for Supported Accommodation including any information or form that you may need.

This register includes information about the person and his or her support needs, the preferred accommodation location(s), and details of available vacancies.


RESPITE CARE

We operate in a 24/7 model of planned or emergency temporary care provided to caregivers of a child or adult with intellectual disability. We provide both flexible and center-based.

Each respite facility house can accommodate 3 to 4 clients. We would like to keep the number of clients in a house to a maximum of 4 to fully support them individually especially having to meet their person-centered goals.

At respite, we give the best experience of spending it at Hale but also the support to strengthen your independent skills.

day program - community access

Community access programs are usually done outside of the centre either at a community venue or an area of the clients’ choice. These activities include church programs on Sundays or during the week. This is an opportunity for our clients and their families to meet members of the community and to participate in programs and activities carried out in integrated settings. The organisation has learnt that these community activities have helped in building their self-confidence and uphold the sanctity of human life.
